This could include multiple different tabs, history and physical, a medications tab, could be a diagnostic result but in this particular example, we have a nurses' note. So it's a six-month-old client. We are in a pediatric unit. The client has increased irritability, has a G tube that is leaking. The client has had this G tube for two weeks due to a diagnosis of failure to thrive. The site of the G tube is covered with a dressing that is saturated with old formula. Upon removal of the dressing, the skin surrounding the feeding tube site has some breakdown where it's noted that there's erythema and some flaking. At the insertion site, there is some thick yellow drainage and the feeding tube itself is loose in the insertion site.

And it was a lot of work for some of them. We were able to collect the data in a secure database from an outside vendor, because we never would have been able to do this. We, inside, did the analysis of the data, but they were able to collect it for us. And then 5 years of site visits again, we had about 31 boards that were able to give us this because not all boards do site visits. And we, again, had about 1278 site visit documents. And these were very valuable too, because what you really need to know is what's going on in the program, number-wise, quantitatively, but then in the background, what might be causing it, or the story of the program.

We talk about the U.S. Public Health Service. We are the commissioned corps of the U.S. Public Health Service. So there is a difference. The Public Health Service actually refers to the, I'll just say, HHS instead of trying to say it all.
