The third and final scoring rule is a rationale scoring rule. This scoring rule applies to items that have tightly paired information, and having a full understanding of that concept requires a justification through a rationale. And the simplest form, you can think of this as a nurse must do X because of Y. These items come in two structural forms, a diad and a triad. Diads are worth 1 point, and a triad is worth 2 points. Here's an example of a drop-down rationale. The client is at highest risk for developing X as evidenced by Y. To earn a point, an examinee must get both the drop-down options correct.

Discussions on con- tinued competence accelerated when the Pew Health Professions Com- mission, asserted that continued competence validation among health care providers is vital to safe prac- tice.(2,3) The Institute of Medicine (IOM), The President’s Advisory Commission on Consumer Protection and Quality in the Health Care Industry and The Citizens Advocacy Center have all advocated for a process that objectively measures competence among the post entry-level health care workforce. (2,3,4,5,6,7,8,9,10,11) NCSBN has recognized the necessity to assess the competence of experienced practition- ers and has been at the forefront in addressing this issue.
